Sperm Bank Selection Steps:
1. The first step will be to determine your CMV status which is a blood test. Cytomegalovirus (CMV) is a member of the very large family of viruses known as herpes (see full explanation under chapter 3, screening requirements). It is important to determine the CMV status of the recipient mother before selecting the donor sperm sample.
2. Next, you will set up an account with your selected sperm bank.
3. Some banks require that your provider completes an authorization form. These forms will be given to you at your TDI Coordination Appointment.
Ordering Your Specimens
1. You will purchase your specimens directly from your selected sperm bank.
2. Many patients order 3-4 samples at a time to save on shipping and handling costs. Usually one sample is sufficient for one insemination per month. Occasionally, your provider may recommend using two samples per insemination. Donor sperm specimens may be stored at SRM for additional storage fees.
Specimen types: Donor semen samples used for TDI must be processed, or "washed", in order to separate the seminal plasma and other cells from the sperm.
- Washed IUI (Intrauterine Insemination) samples are processed by the sperm bank and usually are guaranteed to have at least 10 million motile sperm per vial.
- Unwashed IUI or ICI (Intracervical Insemination) samples must be washed by our SRM Laboratory on the day of your insemination in order for placement into the uterus. Please note SRM's increased cost to wash and prepare these types of samples.
- ART or IVF samples are now being offered by some sperm banks. These samples have lower total motile sperm and may only be used with IVF (in-vitro fertilization).
Shipping Your Specimen to SRM (Seattle Location ONLY)
1. You will first contact your selected sperm bank to arrange for shipment.
2. Samples need to arrive at the SRM Laboratory between 8:00am and 4:00pm Monday through Friday, excluding holidays. SRM will not accept weekend or holiday deliveries.
3. Then, complete the middle patient section on SRM's Donated Sperm Acquisition form which documents your donor ID, number of vials you are shipping, and estimated delivery date. Fax this form to 206-301-5096 Attention, Lab Administrator or email seattlecryo@integramed.com (Please note that you will be given this form at your TDI Coordination Appointment. This form must be completed for EACH shipment even if you are using the same donor).
4. Remember - you must have your specimens delivered to SRM BEFORE the start of you menstrual cycle (the cycle month you want to have your insemination done). For subsequent cycles, vials must be delivered within 3 days of knowing the cycle outcome or start of menses. We cannot schedule your insemination until the specimen has arrived to our Seattle office.
